ENTER YOUR LEMONADE STAND INFO:

Please notify us at least 3 days prior to allow us time to update our list.

*Bank of North Dakota/College SAVE will receive the information on this form and will contact the listed adult regarding the $25 College SAVE deposit.

*A child may submit their lemonade stand for multiple dates throughout the summer. However, each child will only receive one $25 College SAVE deposit and one entry for the final $529 drawings.
